Hospital Visit Request Form

The picture on the left shows the multi-faith Chapel within St John's Hospital, Howden, Livingston. Either Rev John McMahon and Rev Dr Georgina Nelson, who are the hospital chaplains, are available to provide spiritual assistance to patients and staff.

Our ministers, too, would wish to know if you are in hospital. They do their utmost to visit members of St Michael's when they are hospitalised in Edinburgh or Livingston. They are also augmented in this task by Pastoral Care Visitors.

If you would like to inform the ministers and request a hospital visit, please fill in the form below. Bear in mind that they as they are frequently visiting others within the hospital, you should not feel that your request would be inconveniencing the ministers.
